Please help us keep the information in our database current.  We do the best we can to keep the information in our database current. However, there are over 1400 services listed in the database, and service providers' addresses, phone numbers, funded services, etc. change frequently. If you notice any errors or ommissions, we would appreciate hearing about it. Everyone who uses FIND is counting on us to have up-to-date information which will enable them to locate much needed services for their clients or themselves. 

Have you heard about a new program? Or discovered a service that is not in the database? Please let us know about it. Just send an email with the name of the program and any contact information (email address, phone number, physical address) you have to New program.

If you're a new agency applying for inclusion, please click here to take a look at our "Inclusion/Exclusion Criteria" first. If you feel your organization fits this criteria, just fill out and submit the two forms below.

Two forms?! Agency or Program? A quick explanation.. An AGENCY is the operating entity of one or more programs. For example, Community Action is the operating entity and Head Start, Weatherization, and Child Care Resource & Referral are all PROGRAMS of the agency. A smaller agency with a single program will probably use the same information on both forms. Each of the forms provides us with different information, but the most important one is the Program Information Form.

Do I need to fill out BOTH forms? If you are just sending us an update on a service that is already in the database, you can simply enter the program name and new information on the Program Information Form. If this is a new agency and service, we would appreciate your filling out both forms. Tedious? Yes. Necessary? Also, yes. (And very much appreciated!)

How do I get the forms to you? We want to make it as easy as possible... You can fax 503.648.4175, or mail 1001 SW Baseline Street, Hillsboro, OR 97123 the forms to us. If you would like an electronic copy of the forms, just drop us an email at Need forms.

Any updates or corrections you submit should appear on this website within 7-14 days. If you don't see them within that time, please call 503-693-3238 to verify that we received your information.  New programs may take up to 30 days for approval, data entry and uploading.
